New NY Excess Lines Documents

We have updated the Part C: Affidavit by Producing Broker in compliance with the changes made by the NYSID to regulation 41. This new affidavit is required to be used on all excess lines policies in the State of New York effective immediately. We willl no longer be accepting the previous edition of this form.

We have also uploaded the two new Notice of Excess Lines Placement Total Cost Forms. These two new forms replace the Total Cost Form that was used previously. For policies that have no multi-state tax allocation use the NY Cost Form with no tax allocation. For policies that have multi-state tax allocation use the NY Cost Form with tax allocation.
